A good fit?

Is this the right kind of care for you?

You’re recovering from surgery. Rehab without loading up the car — we bring the clinic to your living room.

Your pain outlasted clinic PT. Rushed sessions and hand-me-down exercise sheets didn’t fix it. This is the opposite of that.

You want the doctor, one-on-one. Every visit is a full session with a Doctor of Physical Therapy — never a tech, never split attention.

You’re done with the clinic shuffle. No drive, no waiting room, no rebuilding your day around an appointment.

How we treat

One plan, built from the right tools

Your evaluation decides the mix. These are the tools we build it from — most go deeper on their own pages.

Part of most plans

Manual therapy

Hands-on joint and soft-tissue work — from mobilization to thrust techniques — used to calm pain and restore movement. It works best tied to active treatment, so that’s how we use it.

Targeted relief

Dry needling

A thin monofilament needle calms hypersensitive trigger points that keep muscles guarded — often useful for stubborn neck pain, back pain, and headaches. Every patient is screened first; it isn’t for everyone.

Strength & resilience

The Exercise Program

Customized programming designed and monitored by a Doctor of Physical Therapy — strength, mobility, and cardio built around your goals, with weekly accountability check-ins and direct access to your provider.

Post-op recovery

Knee replacement recovery

One-on-one rehab at home after your knee replacement — no post-surgical car rides, no waiting rooms. We bring the equipment; Medicare is accepted and no referral is required in Florida.

The process

What to expect

  1. Start with the application. The new patient application takes a few minutes — tell us what kind of pain you’re dealing with and what you want back.

  2. We talk fit. We’ll reach out by phone or email to make sure we’re the right fit for what’s going on, then schedule your first session.

  3. Evaluation with a Doctor of Physical Therapy. A comprehensive first look — joints to nerves — to find what’s actually driving your pain, including any images and records you have.

  4. We come to you. Home, office, even your gym — we bring the table and every piece of equipment your session needs.

  5. One-on-one, always. The full session is with your doctor — and we answer all of your questions. We mean all of them.

  6. A plan that outlasts us. Treatment plus the tools to stay better: manual therapy, dry needling when appropriate, and progressive exercise you can keep.

Pricing & insurance

Simple options, told to you upfront

Private pay

$150–$200 per session

Use your HSA/FSA or any other payment method. No doctor’s referral needed.

Medicare

$0 for most beneficiaries

RHS is a participating Medicare provider. Most beneficiaries have a supplemental plan and pay little to nothing for physical therapy.

Insurance

Out-of-network

Most plans have out-of-network benefits — we provide a superbill you can submit to your insurance for reimbursement.

The honest version: outside of Medicare, we don’t work inside the insurance model — it’s what tends to force clinics into split attention and capped visits. Here you spend the full session with a Doctor of Physical Therapy, and you’ll likely need fewer visits overall to reach your goals. Questions about cost? See our good-faith estimate policy or text/call (904) 414-5683.
“My time with Dr Derry was worth every penny. After suffering with pain from a car accident almost 4 years ago Physical Therapy was the key to increasing my strength and decreasing my pain. Dr Derry worked with my crazy working mom schedule and traveled to both my home and place of business.”

Questions

Physical therapy FAQs

No — Florida doesn’t require a referral for physical therapy. If we need surgical notes or imaging, we’ll coordinate with your other providers directly.

Yes — RHS is a participating Medicare provider, and Medicare Part B covers in-home outpatient physical therapy. Most beneficiaries have a supplemental plan and pay little to nothing.

Outside of Medicare, we work out-of-network — the insurance model tends to force clinics into shared attention and capped visit counts. Most plans have out-of-network benefits, and we provide a superbill you can submit for reimbursement.

Private-pay sessions run $150–$200, and HSA/FSA cards are welcome. Medicare patients typically pay little to nothing. Text or call (904) 414-5683 and we’ll walk you through it.

Nothing. We bring the table, bands, weights — everything the session needs. We’ve met patients at home, at work, and at their gyms.

No. Home health is a separate service for homebound patients. This is high-level, evidence-based outpatient care that happens to come to you — one-on-one with a Doctor of Physical Therapy, with your exact plan set after the initial evaluation.

We serve Jacksonville, St. Johns, Nocatee, and Ponte Vedra in-home. A little outside that? Text or call (904) 414-5683 — if you’re close, we can usually work something out.
