New The complete at-home sciatica recovery system. Get the eBook →From the first evaluation to the last visit, your doctor comes to your door, whether that’s Julington Creek, Fruit Cove, or a cul-de-sac off 210.
Our Doctors of Physical Therapy treat patients across northwest St. Johns County, from Durbin Crossing to RiverTown, with no drive into Jacksonville required.
The clinic comes to you Northwest St. Johns County added neighborhoods faster than it added services. From Julington Creek, Aberdeen, or RiverTown, physical therapy has usually meant driving SR-13 or I-95 into Jacksonville, and that drive is often the hardest part of getting care.
Led by Michael Derry, PT, DPT, a board-certified orthopedic clinical specialist, our Doctors of Physical Therapy drive to you with a treatment table and everything else we’ll need, and work one-on-one with you from the first visit to the last.

“From the beginning, he put together a thoughtful treatment plan, but what really stood out was how consistently he adapted it as my recovery progressed. … He taught me techniques and exercises I could use at home, along with ways to relieve soreness when I was having a difficult day. These are skills and habits that will benefit me for the rest of my life.”
“He takes the time to truly understand each patient’s unique condition and develops a customized physical therapy program designed to restore function, improve mobility, and maximize recovery. If you have myelopathy, spinal stenosis, neuropathy, or any other nerve-related condition, I highly recommend contacting Dr. Derry personally.”
For most households here, the hard part of rehab is fitting one more drive into a week already built around a Jacksonville commute and school pickups. In-home physical therapy in St. Johns removes that trip. The session happens where your recovery actually has to hold up: your own home.
That matters most after surgery. After a knee or hip replacement, you shouldn’t have to get into a car twice a week to reach a clinic off 210. We treat post-surgical patients at home from the early days on, and we bring everything the session needs.
Living room or garage Your doctor sets up wherever you have a little floor space and brings everything else.
Early post-op, off the road New knees, hips, and spines can start rehab at home instead of riding to a clinic off 210 or 9B.
Built around your week Sessions slot between the commute, the school run, and the weekend.
Our practice is fully mobile: we bring everything you need, and we’ve treated many clients in the Julington Creek area and along the CR-210 corridor. Home is one option, not the only one.
Prefer your amenity center’s fitness room, the gym where you actually train, or your office so the day doesn’t need a second round-trip? Tell us where your week happens and we’ll treat you there.
Your doctor comes to you, the schedule bends around your week, and every visit is with the same Doctor of Physical Therapy who did your first evaluation.
This is relationship-based care. The doctor who watched you move on day one is the one refining the plan on week six, treating hands-on, adding manual therapy or dry needling where appropriate, and adjusting in real time instead of reading someone else’s notes.
And when something changes between visits, like a flare-up after yard work or a question about a new exercise, you reach your doctor directly.
Most clients here self-pay. It’s the simplest route: one flat rate per session, HSA and FSA eligible, and no doctor’s referral needed under Florida direct access. We’re out-of-network with commercial insurance but can provide a superbill after each visit to submit for out-of-network reimbursement, and most employer plans include those benefits. Medicare is the exception: we’re a participating Medicare provider, and with a supplemental plan, most beneficiaries pay $0 or close to it.

Most of our clients are busy executives, people who value their time, and people who’ve had a frustrating experience with the traditional physical therapy model: driving to a clinic and being treated alongside several other patients at once. Many of the problems they bring us, from backs that seize on the commute to necks worn down by desk work, respond well to focused one-on-one care.
Medicare covers our in-home visits, including rehab after knee replacements, hip replacements, and lumbar fusions, at $0 or close to it for most beneficiaries with a supplemental plan. Our doctors treat with manual therapy, dry needling, and progressive exercise. Post-op patients can see how knee replacement recovery at home works before applying.
